Independent Living

Direct Independent Living Services

  • Strength/Needs Assessment
  • Focused Life Skills Assessment
  • Full Time Life Skills Coaching
  • Transitional Living Plan 
         -  3-4 goals per quarter
         -  One exercise or more per week with the assistance of an In-House Mentor
         -  One documented individual meeting per week
         -  Completion of activities documented on level packet
         -  Transitional Living Plan updated each quarter/attached to Treatment Plan
             and submitted to Referring Agency
  • Facilitates Life Skills Community Meeting once per month
  • Encourages ownership of independent living issues through the weekly Pioneer Club.  This club addresses the leadership program and follows the established pro-social normative culture to transition students into independent living.  

Objectives

  • Raise awareness of the need for transitional living preparation
  • Improve counselors' ability to teach/instruct in independent living skill development
  • Provide instruction and experiences to enable youth to exert control over their own lives and provide opportunities for youth to practice Life Skills in a "real world" environment
  • Ensure that youth understand that acquiring Life Skills is a life-long process
  • For youth to accept full responsibility for their own Life Skill mastery
  • For youth to demonstrate regularly their learned Life Skills
  • To empower youth to appropriately advocate for themselves while developing leadership skills
  • For youth to actively participate in planning their present and future life After System Care

Assessment Tools

YES Academy will utilize "state-of-the-art" Life Skills Tracking, a strength based competency tool, to determine skill mastery in the following nine domains which include 30 Life Skill areas:

  1. Career Planning
  2. Communication
  3. Daily Living
  4. Home Life
  5. Housing & Money Management
  6. Self-Care
  7. Social Relationships
  8. Work & Study Skills
  9. Work Life
